Case Management, Dementia Specific

Dementia specific case management is a specialized, comprehensive, and complex service that involves building a trusting relationship with the client/family/network, to provide ongoing support around dementia related issues; help the client function in the least restrictive, most natural environment and; achieve an improved quality of life (i.e. on-going process of client assessment, service planning, system navigation, care coordination and monitoring).
May include: Standardized assessments (e.g. RAI-HC/CHA, functional assessments, cognitive assessments); Relationship building, care planning and goal setting; Direct service provision/intervention (services aimed to achieve the highest level of functioning possible); In-direct service provision (obtaining necessary services through linking and collaboration with other health and community providers); Monitoring and follow-up; Information and referral; Liaison, advocacy and consultation with other resources based on client's health and social needs.
